• Kirbs Massage Clinic Membership Card is valid for 12 months, remains the property of Kirbs Massage Clinic and is non transferable.

• The Kirbs Massage Clinic Membership Card must be presented every visit. No card, No points, no discount.

• All Discounts will be base on our Regular Rates.

• Membership Discounts cannot be combined with any other offers or promotions.

• Kirbs Massage Clinic reserves the right to change clinic rules, regulations, and pricing at any time upon providing reasonable notice.

• You agree to follow clinic rules and regulations. We have the right to refuse or discontinue service for any reason.

 

 

 

CARD REPLACEMENT AND LOST CARDS:

 

• A membership application form must be filled up to replace a lost or damaged card.

• A replacement fee of fifty pesos (P100.00) will be charged for lost card.

• Points in a lost card cannot be transferred to a new card.
